Absence and Sickness

If your child is unwell and is unable to attend school please contact us at your earliest convenience.  Please either telephone the School Office on 0118 973 3118 or complete the form below, thank you.

You may also use this form to notify us of future absence (doctor's appointments, dentist etc).

Please do not use this form if you are requesting unauthorised absence (eg : holiday during term time).  In this circumstance you must send a letter to Mrs Brown, Head Teacher.
